If your school or district has approved funding for your Breathe For Change program, you’ll need to request an invoice and submit payment.

Request an Invoice

Most organizations will require an invoice in order to generate a purchase order, or to submit payment via check or credit card. To request an invoice for your school or organization, complete the Invoice Request Form.
